BAKUN BOAT HOUSE HOMESTAY DECLARATION FORM & INDEMNITY

All participants must read this Declaration Form/Indemnity Clause and submit it to our representative before we can confirm your booking. For children and minors (below 18 years), a “Letter of Authorization” is required if their legal parent(s)/guardian(s) are not traveling along.

Declaration & Indemnity Clause

I confirm that I am physically fit and mentally capable of participating in the activity or trips in which I will engage, acknowledging the inherent risks involved.
I understand the importance of obtaining travel insurance to cover myself from any unforeseen incidents and accidents during my participation in this trip. I indemnify your company from any responsibility or damages occurred or suffered by me during my participation.
I acknowledge that my participation is voluntary, and I will assume financial responsibility for any personal injury, accidents, illness, and damage to or loss of personal property that may occur.
I understand and accept that weather conditions may necessitate changes to the planned itinerary, and I agree that your company has the right to adjust activities accordingly.
In the event of circumstances beyond control such as riot, political unrest, natural disasters, etc., resulting in the cancellation or change of activity arrangement, I understand that your company shall not be liable for any refunds.
I agree to abide by all rules, guidelines, and protocols provided by your company and/or trip leader throughout the activity or trip.
I understand that my behavior during the trip must not cause distress or harm to myself or other participants. Your company reserves the right to disallow or terminate my participation if necessary, and I agree to make my own arrangements to leave the trip site if requested.
I acknowledge that I am responsible for my own personal belongings and properties throughout the trip, and your company shall not be held responsible for any damages incurred.
